For patients under 18 years of age, we ask that you are accompanied by a parent or guardian. It is also of great importance to tell us if you taking any medication prescribed by your physician or have an existing medical condition. Please continue to take any prescribed medicine at the prescribed time. If applicable, please assist us by providing the following items: existing dental x-rays, a list of prescribed medications you are currently taking, and any dental insurance with completed forms.
